Tutor Application

Please complete and submit an application form if you are interested in becoming a volunteer literacy tutor.

Your completed application will register you for an orientation and add you to our mailing list. If you have questions about the form or the program, please contact the literacy office at quliteracy@ocln.org or 617-376-1314.

Confidentiality Policy
It is the policy of LVM-Quincy to maintain strict confidentiality of student and volunteer records and information. All information, including personally identifiable information, about individual students and volunteers shall not be shared with any entities or parties outside of LVM-Quincy or Literacy Volunteers of Massachusetts without prior written consent from the student and/or volunteers that shall include a list of the information to be released and instruction about its use.
